One of the worst experiences in restaurants is ordering meals prepared as we like and getting something different. Different people react in different ways, and this one is definitely fit for an Oscar, even though some people will argue that the person should have informed the staff about the mix-up.
It is understandable why the customer felt slighted and left a note regarding their dislike for onions in their meal. At least they did not become aggressive and just used the onions to send the message. Still, being polite and clear about your preferences can lead to a better dining experience for everyone involved.
